Video Submission Release Agreement
By checking this box, you confirm that you are the creator and owner of the testimonial/video/photo(s) submitted and that you grant MAKE Wellness the right to use, format, shorten, or adapt your content—without changing its original meaning - for use in connection with our business activities, including marketing and promotions, in any media now known or later developed.
This includes, but is not limited to, our website, social media platforms, printed materials, digital campaigns, and live events. You also confirm that any photos or videos you submit are recent, unaltered, and not edited with filters or enhancements that change your appearance or the appearance of the products. You also grant permission to use your name, likeness, image, and voice as part of this content. You acknowledge that you will not receive compensation for this use, waive any right to inspect or approve the finished product, and release MAKE Wellness from any claims or liabilities related to the use of your content. Please share authentically while avoiding noncompliant health, medical, income, or earnings claims.
